The Pro Bono Civil Surgeon Registry is a self-service directory of USCIS-designated civil surgeons offering pro bono, sliding-scale, or reduced-cost I-693 exams. Once your organization is verified, you receive access to the directory and contact civil surgeons directly to schedule appointments for your clients.

Who the directory serves.

The directory is available to attorneys, DOJ accredited representatives, paralegals, and legal directors at organizations representing immigrant clients in cases requiring an I-693 medical examination.

From access request to exam.

PCSR is a self-service directory. PCSR does not host clinical appointments, collect fees, store medical records, or broker individual referrals. We maintain the directory. Attorneys and civil surgeons coordinate with one another directly.

Submit an access request

Complete the short access request form with your organization, your role, your service area, and the approximate volume of clients you anticipate referring.

PCSR verifies your organization

We confirm your organizational affiliation before granting access. This protects participating civil surgeons from unverified outreach.

Directory access granted

Once verified, you receive access to the directory โ€” a list of participating civil surgeons with practice location, contact information, ages served, languages spoken, and fee arrangements.

Contact civil surgeons directly

Identify a suitable civil surgeon in the directory and contact the practice directly to schedule the exam for your client. PCSR is not involved in scheduling or case coordination.

Practical guidance for a smooth exam.

Most I-693 exams, once scheduled, can be completed in a single visit. A few notes that help the process go smoothly:

Identification

Your client will need government-issued photo ID (passport or state ID). Civil surgeons are required to verify identity.

Vaccination records

If your client has existing immunization records, bring them. Missing vaccines will typically be administered at the exam.

Interpretation

Many participating civil surgeons have in-house language capacity; for less common languages, arrange interpretation in advance.

Sealed form

The completed I-693 must remain sealed in its original envelope until submitted with the I-485. Do not open it.

Validity

A signed I-693 is generally valid for USCIS filing for two years from the date of signature.

Directory updates

If a civil surgeon in the directory is no longer able to accept pro bono appointments, or if you notice an entry that needs correction, email PCSR so we can keep the directory accurate for everyone.

A growing national network.

PCSR currently includes civil surgeons across multiple regions of the United States, with particular depth in the Midwest and ongoing recruitment in areas of greatest need. The directory is continuously updated as new civil surgeons enroll.

If your client’s state or metropolitan area is not yet represented in the directory, submit the access request anyway. PCSR uses demonstrated legal-services demand to prioritize where we recruit next, so your request itself is useful data even when no current listing fits your client.
